Manager check-ins should address their SWOT on their performance as a manager as well as each of their areas of responsibility. Each check-in should have key take-aways: specific and measurable goals to help them reach their opportunities, as well as a plan of action to address weaknesses and potential threats.
You may use the Manager Check-in Template or the Manager FEEDBACK Template, or if you prefer all notes can be typed or handwritten so long as the discussion contains actionable take-aways.
PLEASE NOTE: your manager should be given a copy of your check-in / eval notes, ensuring they have a specific set of goals and actionable behaviors to work on. Print or make a copy of your notes for the manager to keep, but please do not include pay rates on the copy.
